Appointment Process

Please call (909) 969-1111. Dr. Connie Johnshoy-Currie will speak with you personally to determine how best to assist you with your needs. She will then schedule your initial appointment.

Appointments are scheduled on a reserved, standing basis, whenever available. If you cannot reach Dr. Johnsoy-Currie, you can fill out the wait list form below.

 

If this is an urgent or emergency situation, please call 911 or go to the nearest emergency room. Messages left via this waiting list will not receive an urgent timely response.

Because your appointment time is reserved exclusively for you, a minimum of 24 hours notice is required for a cancellation, in order not to be charged your usual appointment fee. Appointment cancellations are to be made on the (909) 969-1111 number.​
